Dry needling uses a thin, solid filament needle inserted directly into a taut band of muscle. It is called dry because nothing is injected — the needle itself is the treatment.
The aim is a local twitch response: a brief, involuntary flicker of the muscle as the needle reaches the tight band. That twitch is associated with the muscle releasing, and patients typically describe the sensation as a deep ache or cramp that fades within seconds.
Intramuscular stimulation, or IMS, is a specific systematised approach to needling developed around the idea that persistent muscle tightness often has a neuropathic component — that is, an irritated nerve driving muscle shortening. In practice the terms are often used interchangeably, and the technique your physiotherapist uses will be matched to your presentation.
Needling reaches muscle that is genuinely hard to influence by hand: deep gluteal muscles, the deeper layers of the calf, muscles lying under other muscles. That is its main advantage over manual work.

Needling is not a standalone service here. It follows an assessment, and it is used where the examination indicates it will help — alongside manual therapy and a loading program, not instead of them. Needling a muscle that keeps being overloaded gives temporary relief and nothing more.
Needles are single-use and sterile, and are disposed of immediately. Treatment usually takes only a few minutes of the appointment.
Insertion is usually barely felt. What people notice is the twitch response — a deep, brief cramping ache. Most describe it as strange rather than painful, and it passes in seconds.
Muscle soreness for 24 to 48 hours is normal and expected, similar to after a hard workout. Gentle movement, warmth and water help. Occasionally mild bruising occurs.
In trained hands, yes. Physiotherapists performing dry needling in Alberta complete additional certification, and technique around anatomically sensitive regions is a core part of that training. Tell your physiotherapist about blood thinners, bleeding disorders, pregnancy, a compromised immune system or a pacemaker — these change what is appropriate.
Many people notice a change after the first or second session. Needling is generally used for a short course while the underlying strength and movement work does the durable part.

Dry needling is a useful tool, not a universal one. We would not lead with it where the primary problem is joint rather than muscle, where symptoms point to a nerve compression that needs different management, where you have a needle phobia significant enough that the treatment would be counterproductive, or where a medical condition makes it inappropriate.
In those situations we will say so and use something better suited. An honest no is more useful to you than a treatment that was never likely to work.
