Where the Evidence Actually Sits
Low back pain is where spinal manipulation has its best support. Clinical guidelines in Canada and elsewhere generally list manual therapy, including spinal manipulation, among the recommended options for non-specific low back pain, particularly alongside staying active and exercise.
Two things are worth being straight about. First, manipulation is one reasonable option among several rather than uniquely effective. Second, the combination matters more than any single technique — treatment plus movement plus not resting for too long consistently beats passive treatment on its own.

What We Assess
Low back pain is not one condition, and treatment depends on which one you have. Assessment sorts that out:

Mechanical low back pain
Pain that changes with position and movement, without nerve involvement. The most common presentation and the most responsive.

Sacroiliac and pelvic pain
Typically one-sided, low, and worse with single-leg loading such as stairs. See chiropractic for sciatica where it refers down the leg.

Disc-related pain
Often worse with sitting and bending, sometimes with leg symptoms. Managed differently and more cautiously.

Facet joint restriction
Localised, often worse with extension and rotation.

Muscular and load-related
Where the driver is deconditioning or a sudden change in physical demand.

Something that is not mechanical
The reason assessment includes screening. Some back pain needs a physician, not a chiropractor.
What Treatment Looks Like
Adjustment where it is indicated, soft tissue work around it, and a small number of exercises you actually do. Most mechanical low back pain improves over a handful of visits. If it is not changing, that is information — it means the working diagnosis needs revisiting rather than the same treatment repeated.
We will also be direct about what makes the difference between recovering and recurring: staying active rather than resting, addressing whatever load caused it, and building capacity in the muscles that support the spine. Therapeutic exercise covers that side.
